Team’s rapid response keeps man out of hospital

A MAN avoided an unnecessary return to hospital thanks to the swift actions of Bluebird Care Reading, Wokingham, Windsor, Maidenhead and Bracknell team members, following an urgent request from the NHS Continuing Healthcare team.

The call came just hours after he had been discharged. Without immediate care at home, his discharge would have failed, forcing him back into hospital, taking up a much-needed bed and potentially slowing his recovery.

Registered care manager Kerry Brooker and care supervisor Becky Underwood (pictured) wasted no time. They collected their tunics and PPE and headed straight to his home. Within minutes they were by his side, delivering the essential care, reassurance and professional support required to keep him safe and well.

“It’s just what we do,” said Kerry. “We want people to get the best possible start after leaving hospital. No one wants to see them go back in if we can help it.”

The quick intervention did not just help one individual – it also eased pressure on local NHS services, ensuring a hospital bed was available for another patient in need.

Bluebird Care works in close partnership with the NHS to support CHC-funded individuals, enabling safe, timely hospital discharges and helping prevent avoidable readmissions.
